 Last month Dr Deshpande came down from Gillettes children hospital USA (one of the oldest childrens hospital in USA) and like last year both Dr Vipul Shah and Dr Deshpande with their team examined difficult cases of cerebral palsy and developmental disabilities together for 2 days in Delhi

Around 40 children were comprehensively examined ,thanks to the R.P,Shah Memorial trust for Children with Disabilities ,Asio African Academy of Cerebral Palsy and Reaching for the Stars-Asian Initiative for the logistic support for the endeveour

dr shah taugh orthopaedic colleagues the criteria for  best managing forearm fractures in children, presented the evidence on the use of botulin in cerebral palsy and  how to do growth modulation surgery in children.growth modulation surgery is used to eqalize the lenghts of the leg bones when one leg is bigger or smaller than the other and is much superior procedure than the old procedure which involved cutting  of bones,incidentally  dr shah was taugh this technique first hand  by dr peter stevens  himself who invented the  8 plates when dr shah spent 4 months at the primary children hospital salt lake city utah,usa in 2007.

Gillettes children hospital,USA  R P Shah memorial trust and Reaching for the Stars,USA( Asia Initiative) got together and in their efforts to bettter quality of care for children with cerebral palsy organised a camp 


the foresaid camp ran from 1st jan to 6th january  at lucknow where the gillette team and dr vipul shah together examined 200 patients of cerebral palsy free of cost ,every day the doctors used to start opd at 8 in the morning and used to work non stop till 10 pm at night often without gettting time for food or rest,the gillete team was lead by dr deshpande and dr vipul shah coordinated for r p shah memorial trust and reaching for the stars initiativethanks to the team for making this possible and thanks to the trustees of R P SHAH MEMORIAL TRUST for making all arrangements

 R P Shah Memorial Trust for Children with Disability and Reaching for the Stars -Asia Initiative together with the ICF research branch at Switzerland and the WHO centre for Germany together came on one platform to do the ICF training course at kochi ,india


Dr Vipul shah ,Pediatric Orthopaedic Surgeon and Medical Director of the trust was the organizing chairperson.the workshop created the first group of 25 professionals in india who have the requisite know how to use the ICF and code it


Information about functioning and disability is essential for clinical practice, research, education, as well as for health, social policy and advocacy. Increasingly, countries are enhancing their data about functioning and disability using the WHO International Classification of Functioning, Disability and Health (ICF). The ICF is an international standard for health and disability information - key for collecting valid, reliable and comparable health and disability data.


 This two-day interactive training comprises of lectures and exercises (single and group work) aimed at training

2 children of scoliois who were so poor not to be able to afford a brace were promptly adopted by dr vipul shah and will be followed up free by him for life ,in addition their cost of bracing throughout life will be borne by him personally,in addition 2 very poor children from badohi with club feet will also be treated free of cost by dr shah


one very poor child with scoliosis who suddenly lost his power in his legs and also suddenly lost urine control and will need a 10 hour surgery to reduce the problem was immediately adopted by the r.p.shah memorial trust for children with cerebral palsy and the whole cost of his 10 hour surgery will be borne by the trust with the help of well wishers

CLUB FOOT BRINGS A CHILD FROM BOSTON(USA) ALL THE WAY TO LUCKNOW IN INDIA WITH DR VIPUL SHAH,PEDIATRIC ORTHOPEDIC SURGEON

this is the story of sonia and kuntal in boston (USA),blessed with a beautiful baby boy only to find that sattaki had bilateral club feet,in which both his feet were turned inwards,after numerous consulatations sattakis parents started him on plaster in boston, 7 plasters later sattaki was still looking as deformed as earlier,sonia and kuntal started getting desperate and found about the indian cerebral palsy team and dr vipul shah,dr shah one of the very few surgeons in india using the ponsetti technique looked at the e mailed pictures and told sonia that sattaki could be corrected, sonia came down to lucknow and within just two plasters sattaki was fully corrected ,none of the casts slipped from sattakis leg much to the delight of sonia and kuntal and 2 months down the line sattaki is fully cured and sonia is  back to boston ,we wish sattaki a great future and dr shah will be following sattaki up regularly by images and you tube videos!!

In what has been widely appreciated as  a measure of our committment towards children/young adults with scoliosis miss roli dixit who had been operated for scoliosis (130 degree curve ) by dr vipul shah joins the indian cerebral palsy team as a counsellor, miss roli was operated a year ago and before surgery was barely able to walk 100-150 metres and barely able to sit for an hour because of pain hasnow become fit after surgery -her braces are off,she is able to sit for 8-10 hours at a stretch without any pain and is able to walk for long distances upto 5 km without any problem,dr shah had before surgery invited rolli to join the team which she did ,she will continue to mentor patients and also talk to parents who have children with scoliosis helping them navigate this tough condition

2 NEW RESEARCHERS JOIN INDIAN CEREBRAL PALSY TEAM

in a recent development dr garima singh and dr shanu verma joined the indian cerebral palsy team ,they will undergo 6 weeks of training before they start their research into the sociological patterns associated with cerebral palsy and the rates of depression in caregivers-primary as well as secondary


this research is a joint collaboration between indian cerebral palsy and the department of sociology DAV COLLEGE ,KANPUR headed by dr manindra kumar tiwari,the team will do a PAN-INDIA SURVEY and the background ground level logistic support will be provided by SAMVAD a NGO headed by Dr abhay srivastava

 
10 year andrew gets a straight back after 8 hours surgery his scoliosis and kyphosis corrected by dr vipul shah

andrew was born with achondroplasia and slowly over the years developed scoliosis and kyphosis,he also had sleep apnoea ( a condition in which your oxygen levels go down in sleep) and problems in his lungs in addition to his achondroplasia ,his parents lesie and shiela were always concerned with his progressive deformity in the back and it was painful  to see their son in this way,leslie and shiela live in sharjah and work in dubai as teachers,they saw various experts all around the globe who would either say that since his lung condition is so bad that nothing could be done or that andrew would need a surgery right away but his chances were 50% ,they were always  on the look out for someone whom they in their own words trust their child onto luckily they met dr shah,dr shah trained at the best centres in usa and an expert in scoliosis treatment was like a breath of fresh air and he initially put andrew aged 10 years over to strict regimen of therapy to make his body withstand the surgery,4 days before the due date both leslie and shiela brough andrew back for surgery but he had in the interim developed a little cold so the surgery was delayed for a week while the pediatrician dr santosh ,anaesthetist dr rishi and the pulmonologist had look at andrew to fight the cold and making him fit,thanks to the therapy andrew was fit in a week and was taken up for surgery on 24th december,the surgery was planned and andrew was wheeled while everyone prayed,dr shah and his team first placed andrew prone on his tummy ,opened his back and straightened him and put graft in ,then they found that andrew was doing well and could withstand the second stage so they closed the back and placed andrew on the side ,over the next four hours dr vipul shah opened andrew from the front and placed grafts to keep andrew straight,at the end of surgery andrew who`s angle was 110degree before surgery was already 30  degree post surgery (which lasted for 8 hours)


andrew was out of surgery and onto his normal bed thanks to preop therapy and a few naturopathy medications that dr shah had prescribed he did not go into ICU(intensive care unit) nor did he need any ventilator support


leslie and shiela saw and were with andrew immediately after the surgery which they termed as amazing because everywhere they had gone they were told at least 10-15 days in intensive care unit,they termed this as their best ever  CHRISTMAS PRESENT

women should avoid soft drinks during pregnancy

Pregnant women should avoid artificially sweetened diet soft drinks, as a study has linked them with premature births.



Research conducted on almost 60,000 pregnant women in Denmark found that those who drank such soft drinks, whether fizzy or still, were more likely to give birth early.



It was found that those who drank one serving daily of artificially sweetened fizzy drink were 38 percent more likely to give birth before 37 weeks




Those who consumed 4 servings/day were 78 percent more likely to have their baby prematurely, according to the American Journal of Clinical Nutrition.



It is suspected that artificial sweeteners are broken down in the body into chemicals which may affect the womb, the researchers said.



The study was conducted by Thorahallur Halldorsson of the Statens Serum Institute in Copenhagen, Denmark

new drug for drooling in children

Drooling is normal in infants. But a significant proportion of the developmentally disabled population experiences drooling caused primarily by neuromuscular dysfunction that makes it hard to swallow. Cuvposa reduces drooling by lowering the volume of saliva produced.


Glycopyrrolate was approved decades ago to treat peptic ulcers and reduce salivation in patients under anesthesia. Until now, glycopyrrolate has been used on an off-label basis to treat drooling in the developmentally disabled population, but in a different dosage form than the approved product. A drug is said to be used off-label when a physician prescribes its use in a different way than described in the FDA-approved drug label.


In 2001, the FDA held an advisory committee meeting to discuss how best to develop products for drooling with ethically and scientifically sound trials in children who have neurological disorders. Utilizing the advice provided, the FDA has been able to move forward in addressing the needs of this population.


“Cuvposa provides an important therapy for controlling salivation in patients with neurologic disease,” said John Jenkins, M.D., director of the Office of New Drugs in the FDA’s Center for Drug Evaluation and Research. “FDA approval not only ensures that the product meets modern standards for safety, effectiveness, quality and labeling; but, also results in a more suitable dose form for this patient population.”


The FDA drug approval process provides a review of product-specific information that is critical to ensuring the safety and efficacy of a finished drug product. For instance, the applicant must demonstrate that its manufacturing processes can reliably produce drug products of expected identity, strength, quality, and purity.  .


When used off label, oral tablets of glycopyrrolate had to be crushed to treat drooling in children with neurological disorders. Cuvposa is a flavored oral solution that is easier to administer and provides the optimal dose for each patient.


In clinical trials of Cuvposa glycopyrrolate oral solution, 78 percent of the children on the drug reached clinical improvement in drooling compared with 19 percent of those given an inactive substance (placebo).


Common adverse reactions reported with glycopyrrolate are dry mouth, constipation, flushing, and urinary retention
